Note
  • Interviewed by Barbara Sellers-Young on June 29 or 30, 2002 in Amsterdam, Netherlands (cassette 1) and on March 26 and 27, 2001 in Sacramento, Calif. (cassettes 2-4).

  • Cassette 1, taped June 2002, is an attempt to recreate an inaudible March 2001 interview.
